Insurance, Fees, Billing and Hours

INSURANCE:

I am a provider for VHAP, Medicaid, Cigna, BCBS, Magellan, MVP, AETNA & CBA.  If you have some other insurance and would like to know whether I am covered by your insurance, contact your insurance company directly.

If you do not have insurance or would prefer not to use your insurance, I’d be happy to discuss a sliding fee with you.

FEES:

Initial Consultation (first appointment): $160 (50-75 minutes)

Individual, Couple, Family Session: $110 (45-50 minutes)

Phone Session (via Skype): $25/$50/$110 (15/30/50 minutes)

Email Answer to a Question : $15

Missed/Cancelled (less than 24 hrs. notice):  $110 (not covered by insurance)

BILLING:

My bookkeeper, Karolyn Micheels of Claims Connection, can answer any questions related to insurance and billing.  She takes care of sending out invoices/receipts for co-payments and sends claims to insurance companies.  If you have any questions about your insurance, feel free to email her at ClaimsConnection@charter.net or you can call her work number: 1-800-775-1585

Payment can be made by check, cash or PayPal.  I currently do not accept any credit cards.

HOURS:

Monday: 10-6

Tuesday: 10-6

Wednesday: 2-6

Thursday: 8:45-6

Friday: 10-4:30

I do have some later times available upon request.
